Ozempic is often high-cost without insurance coverage. With a type 2 diabetes diagnosis, many commercial plans cover it. For weight loss patients without diabetes, Ozempic is usually not covered. Wegovy has its own weight-management insurance pathways and savings programs.
Important distinction: Ozempic (semaglutide) is FDA-approved for type 2 diabetes only, not weight loss. For weight management, the FDA-approved semaglutide product is Wegovy.
List price
Without insurance, Ozempic is typically expensive at retail pharmacies. Final out-of-pocket cost varies by dose, pharmacy, and eligibility for savings programs.
Insurance coverage
Ozempic insurance coverage depends entirely on your diagnosis:
- With type 2 diabetes: Many commercial plans cover Ozempic as a diabetes medication, though copays and formulary rules vary by plan.
- Without diabetes (off-label for weight loss): Most plans will not cover Ozempic. Without the diabetes diagnosis code, the claim will be denied. You’d pay the full retail price.
This is the primary financial reason to use the right medication for the right indication. If your goal is weight loss, Wegovy has dedicated weight-management coverage pathways.
Savings programs
Novo Nordisk offers an Ozempic savings card for eligible commercially insured patients with a type 2 diabetes prescription. Savings vary by plan and program terms.
Key limitations:
- Not available for Medicare, Medicaid, or government insurance
- Requires a type 2 diabetes prescription (not off-label weight loss)
- Income and eligibility requirements apply
Note: PEAK accepts Anthem, Cigna, Aetna, UnitedHealthcare, Sentara, and TRICARE commercial plans. We do not accept Medicare or Medicaid.
Cost comparison: Ozempic vs. Wegovy
The list prices are broadly similar. The real difference is in coverage:
- Ozempic: Covered with diabetes diagnosis. Not covered for weight loss.
- Wegovy: Covered by many commercial plans for weight management with prior authorization. Has its own savings card program specifically for weight loss patients.
For weight loss patients, Wegovy is often the more affordable option because it’s on the correct formulary.
PEAK’s approach to cost
At PEAK, we verify your insurance benefits before your first visit. This means you know your out-of-pocket cost for Wegovy, Zepbound, or any other medication before starting treatment. We also help with prior authorization, savings card enrollment, and manufacturer assistance programs.
We don’t prescribe Ozempic for weight loss because it creates unnecessary financial risk. Using the FDA-approved weight loss medication gives you the best chance at coverage. Schedule a consultation to check your coverage.
Boxed warning — thyroid C-cell tumors: Semaglutide (Ozempic/Wegovy) carries an FDA boxed warning for thyroid C-cell tumors observed in rodent studies. It is contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN2). Tell your provider immediately if you notice a lump in your neck, difficulty swallowing, or persistent hoarseness.
