How to become a new patient
The easiest way to get started is to fill out our online intake form. This lets our team begin verifying your insurance coverage right away — often before we even speak on the phone. The sooner we have your information, the sooner we can let you know what your plan covers and get you scheduled.

Our front desk team will ask a few basic questions about your health history and goals, collect your insurance information, and walk you through what to expect at your first visit, including what to bring and what your estimated out-of-pocket costs will be.
If you have questions about specific medications like Wegovy or Zepbound, our team can provide general information about our prescribing approach. For detailed clinical questions — such as whether a particular medication is right for your situation — those are best addressed during your first visit with your provider, who will have your full medical history in front of them.
Insurance verification before your first appointment
After your initial call, our team contacts your insurance company directly to confirm your plan details — office visits, nutrition counseling, lab work, and GLP-1 medication coverage including any prior authorization requirements. You will know your estimated copay, coinsurance, and deductible responsibility before your first appointment.
We accept Anthem, Cigna, Aetna, UnitedHealthcare, Sentara, and TRICARE (commercial plans). We do not accept Medicare or Medicaid, including Medicaid managed care plans. For a full list, visit our insurance page.

Questions patients ask before their first visit

Do I qualify for medical weight loss? Generally, patients with a BMI of 30 or higher, or a BMI of 27 or higher with a weight-related health condition such as type 2 diabetes, hypertension, or high cholesterol, may be candidates for GLP-1 medications. Your provider will make the final determination at your first visit based on your full medical history and a physical examination.
How long will my first appointment take? Plan for approximately 45 to 60 minutes for your initial visit. This includes a thorough medical evaluation, discussion of your goals and medical history, lab review, and development of your individualized treatment plan. Follow-up visits are typically shorter.
What should I bring? Bring your insurance card, a photo ID, a list of current medications, and any recent lab results if you have them. If you have records from a previous weight loss program, those can be helpful but are not required.
Do you prescribe compounded semaglutide? No. PEAK prescribes only FDA-approved medications — brand-name Wegovy (semaglutide) and Zepbound (tirzepatide). We do not prescribe compounded GLP-1 medications under any circumstances due to safety concerns documented by the FDA.
Can I do everything via telehealth? Your first visit must be in person at our Chesapeake office. After that, we are generally comfortable conducting follow-up visits via telehealth as long as you can provide your current weight and blood pressure. Many of our established patients prefer this option for the convenience.
