Is Mounjaro Causing Hair Loss?
Tirzepatide does not directly damage hair follicles. What causes hair thinning is rapid weight loss—and tirzepatide, producing up to 22.5% weight loss, creates conditions for temporary shedding called telogen effluvium.
The same pattern occurs after bariatric surgery or any rapid weight change. In SURMOUNT trials, hair-related events were reported at low rates, consistent with the degree of weight loss rather than a direct drug effect.
Understanding Telogen Effluvium
Hair follicles cycle through growth (anagen), transition (catagen), and rest/shedding (telogen) phases. Physiological stress—including rapid weight loss—shifts more follicles into telogen simultaneously, causing diffuse shedding 2–4 months after the triggering event.
This delay explains why patients notice shedding months into treatment—it’s a response to weight loss that began earlier, not yesterday’s dose.
Telogen effluvium is temporary and self-limiting. Once weight stabilizes and nutrition is adequate, regrowth follows within 3–6 months.

When Does Hair Regrow?
- Peak shedding: 3–6 months after rapid weight loss begins
- Shedding duration: 3–6 months total
- Noticeable regrowth: 3–6 months after shedding stops
- Full recovery: 6–12 months after shedding resolves
Weight stabilization is the most important factor. See a dermatologist if hair loss is patchy, severe, or continues 6+ months after weight stabilization.
Boxed warning — thyroid C-cell tumors: GLP-1 receptor agonists (semaglutide, tirzepatide, liraglutide) carry an FDA boxed warning for thyroid C-cell tumors observed in rodent studies. They are contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN2). Tell your provider immediately if you notice a lump in your neck, difficulty swallowing, or persistent hoarseness.
