Why Tirzepatide Causes Constipation

Constipation on Mounjaro results from multiple interconnected mechanisms:

  • Delayed gastric emptying: Slowed GI transit throughout the entire tract
  • Reduced food volume: Less bulk moving through intestines means less peristaltic stimulation
  • Dehydration: Reduced appetite leads to reduced fluid intake from both beverages and food
  • Lower fiber intake: Appetite suppression leads to simpler, lower-fiber food choices

Timing: Constipation tends to be worst during dose increases (especially 5–7.5 mg) and often improves as your body adapts over 4–8 weeks at each dose.

  • Soluble fiber: Oats, chia seeds, psyllium husk—forms a gel that softens stool

Boxed warning — thyroid C-cell tumors: GLP-1 receptor agonists (semaglutide, tirzepatide, liraglutide) carry an FDA boxed warning for thyroid C-cell tumors observed in rodent studies. They are contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN2). Tell your provider immediately if you notice a lump in your neck, difficulty swallowing, or persistent hoarseness.
