Your first injection can feel like a big moment. You have the pen in your hand, you have read the instructions, and now you actually have to do it. The good news is that it is far simpler than most patients expect. The Wegovy pen is designed for self-injection at home, and the entire process takes less than a minute.
This guide walks you through everything — from understanding the pen device to what to expect in your first week. Our clinical team uses this same walkthrough when we train patients in-office, so you are getting the same guidance here that you would in person.
Before your first injection
Wegovy comes as a prefilled, single-use pen. There is no mixing, no loading cartridges, and no drawing medication from a vial. The dose is pre-measured and sealed inside the pen. Each pen is used once and then disposed of.
Understanding the pen device
The Wegovy FlexTouch pen has a few key components you should be familiar with before your first use:
- Pen cap. Covers the needle when not in use. You will remove this just before injecting.
- Dose window. Shows the dose indicator. For Wegovy, the dose is pre-set — you do not need to dial anything.
- Dose button. The button you press to deliver the injection. You will press and hold this during the injection.
- Needle (hidden). The needle is built into the pen and is not visible until the cap is removed. It is a thin-gauge needle designed for subcutaneous injection.
Storage before first use
Before you use it, your Wegovy pen must be stored in the refrigerator at 36°F to 46°F (2°C to 8°C). Do not freeze it. Do not store it near the freezer compartment. If a pen has been frozen, do not use it — discard it and use a new one.
Take your Wegovy pen out of the refrigerator approximately 30 minutes before you plan to inject. Injecting cold medication can be more uncomfortable. Letting it warm to room temperature makes the injection smoother and reduces the chance of stinging at the injection site.
Step-by-step injection process
Once your pen has reached room temperature, follow these steps. The entire process takes less than a minute.
- Check the expiration date. Look at the label on the pen. Do not use the pen if the medication has expired or if the solution looks cloudy, discolored, or contains particles.
- Wash your hands. Use soap and water. Dry them thoroughly.
- Choose your injection site. Select an area on your abdomen (at least 2 inches from the navel), the front of your thigh, or your upper arm. More on site selection below.
- Clean the injection site. Use an alcohol swab and let the area dry completely before injecting.
- Remove the pen cap. Pull the cap straight off. You will see the needle tip. Do not touch it.
- Place the pen against your skin. Hold the pen at a 90-degree angle against the injection site. Press firmly so the pen sits flat against your skin.
- Press and hold the dose button. Press the button with your thumb and keep holding it down. You will hear a click when the injection starts.
- Wait for the second click. Keep the pen pressed against your skin. You will hear a second click, and the dose window will show the dose indicator has moved. Continue holding for a slow count of six seconds after the second click to ensure the full dose is delivered.
- Remove the pen. Lift the pen straight away from your skin. The needle will retract automatically.
Releasing the dose button or pulling the pen away too early. If you do not hear the second click or see the dose indicator move fully, you may not have received the complete dose. Hold the pen against your skin for the full count. It is better to hold a few seconds too long than to pull away too soon.
Injection site selection
You have three approved areas for Wegovy injections. Each one is equally effective — the medication works the same regardless of where you inject it.
- Abdomen. The area around your belly button, at least 2 inches away from the navel in any direction. This is the most popular site for most patients.
- Front of the thigh. The middle and outer area of the front of your thigh. Avoid the inner thigh and the area directly above the knee.
- Upper arm. The back or outer area of the upper arm. This site may require assistance from another person, as it can be difficult to reach on your own.
Rotate your sites
Do not inject in the same spot every week. Rotating injection sites allows the tissue to recover and reduces the risk of irritation, lumps, or skin changes at the injection site. A simple rotation pattern works well — for example, right abdomen one week, left abdomen the next, right thigh the week after, and so on.
Do not inject into areas with bruising, scars, stretch marks, or skin irritation. Do not inject into hard or lumpy areas. These areas may affect how the medication is absorbed and can increase discomfort. When in doubt, choose a clean, smooth area of skin with a small amount of subcutaneous fat.
What it feels like
This is the question every new patient asks, and the honest answer is: it is less than you are imagining. Most patients describe the sensation as a brief pinch that lasts one to two seconds, followed by a feeling of slight pressure as the medication is delivered.
The needle on the Wegovy pen is a thin-gauge needle designed specifically for subcutaneous injection. It is shorter and thinner than the needles used for blood draws or vaccinations. Many patients report that they barely feel it.
The anticipation is almost always worse than the injection itself. Most patients are surprised by how little they feel.
A few things that can affect how it feels:
- Cold medication. If the pen is still cold from the refrigerator, the injection may sting slightly. Let it reach room temperature first.
- Injection speed. Pressing the dose button slowly and steadily (rather than jabbing) results in a more comfortable experience.
- Site selection. Areas with more subcutaneous fat tend to be less sensitive. The abdomen is generally the most comfortable site for most patients.
- Relaxation. Tensing the muscles around the injection site can increase discomfort. Take a breath and relax the area before pressing the button.
After injecting
Once you have completed the injection, there are a few simple things to take care of:
- Dispose of the pen properly. Place the used pen in an FDA-cleared sharps disposal container. Do not throw it in the regular trash. If you do not have a sharps container, use a heavy-duty plastic container with a tight-fitting lid (like a laundry detergent bottle). Your PEAK care team can provide a sharps container at your first visit.
- Do not recap the pen. The needle retracts automatically. Attempting to replace the cap after use is unnecessary and increases the risk of needle stick.
- Note your injection site. Write down or make a mental note of where you injected so you can rotate to a different site next week. Some patients use their phone calendar or a simple notebook.
- Set a reminder for next week. Wegovy is a once-weekly injection. Set a recurring alarm or calendar reminder for the same day and approximate time each week. Consistency helps your body adjust to the medication.
A small amount of bleeding or a tiny bruise at the injection site is normal and not a cause for concern. If you notice significant bleeding, persistent pain, or signs of infection (redness, warmth, swelling that worsens over days), contact your PEAK care team.
Common first-injection concerns
Every patient has questions after their first injection. Here are the ones we hear most often:
What if I mess up the injection?
The pen is designed to be straightforward. If you pressed the button and heard the clicks, you almost certainly received the dose. If the dose indicator moved to the completion mark in the window, the medication was delivered. If you are unsure, do not inject a second pen — call your PEAK care team and we will walk through what happened.
What if the injection site bleeds slightly?
A small drop of blood or a tiny bruise at the injection site is completely normal. Apply gentle pressure with a clean cotton ball or gauze for 10–15 seconds. Do not rub the area aggressively. The bleeding should stop quickly.
What if I don’t feel anything afterward?
That is expected, especially at the starting dose. You are beginning at 0.25mg, which is the lowest dose in the titration schedule. Many patients do not notice significant appetite changes or side effects at this dose. The medication is working — the effects become more noticeable as the dose increases over the coming weeks.
What if I feel nauseous right away?
Some patients experience mild nausea within the first day or two of their injection. At the 0.25mg starting dose, this is usually mild. Storage and handling
Proper storage ensures your medication remains effective. Here is what you need to know:
- Before first use: Store in the refrigerator at 36°F to 46°F (2°C to 8°C).
- Do not freeze. If a pen has been frozen, discard it. Freezing damages the medication.
- Room temperature storage: An unused pen can be stored at room temperature (up to 86°F / 30°C) for up to 28 days. After 28 days at room temperature, discard the pen even if it has not been used.
- Keep out of direct sunlight. Do not leave the pen in a hot car, near a window, or in any location where it could be exposed to extreme heat.
- Keep the cap on. When stored, keep the pen cap on to protect the needle and the medication from light.
Storage instructions can be updated by the manufacturer. The guidelines above reflect current recommendations, but always check the printed information that comes with your pen or ask your PEAK care team if you are unsure about storage conditions.
Your first week on Wegovy
Your starting dose is 0.25mg per week. This is the lowest dose in the Wegovy titration schedule, and it is intentionally low. The purpose of this dose is to let your body begin adjusting to semaglutide with minimal side effects. You will stay at 0.25mg for four weeks before your clinician increases the dose.
What to expect
At the 0.25mg dose, most patients notice very little change. Some patients report mildly reduced appetite. Some notice nothing at all. Both responses are completely normal. The medication is establishing a baseline in your system — the more noticeable effects on appetite and satiety come at higher doses.
Possible experiences during week one:
- Mild nausea. The most common side effect, but at 0.25mg it is usually minimal if present at all.
- Slight reduction in appetite. You may notice you feel full a bit sooner at meals.
- Mild constipation or changes in bowel habits. Semaglutide slows gastric emptying, which can affect digestion.
- Injection site reaction. A small red area, mild itching, or slight bruising at the injection site. This typically resolves within a day or two.
- No noticeable effects. Many patients at 0.25mg report feeling exactly the same as before. This is normal and does not mean the medication is not working.
First-week tips
Hydrate well. Aim for at least 64 ounces of water per day. Sip steadily throughout the day rather than drinking large amounts at once.
Eat smaller meals. Even before you feel the full appetite effects of the medication, starting with smaller, more frequent meals helps your body adjust.
Keep a log. Note how you feel each day — appetite, energy, any side effects. This information is valuable for your clinician at your next check-in.
Do not expect dramatic changes yet. The 0.25mg dose is a ramp-up phase. Results build over weeks and months as the dose increases.
