Before You Start
Storage
- Keep pen refrigerated (36°F–46°F) until use
- Never freeze—freezing damages the medication
- Check expiration date and confirm solution is clear
- An unused pen can be stored at room temperature (up to 86°F) for up to 21 days
Choose Your Day
Mounjaro is once weekly. Popular choices:
- Friday evening: Any side effects occur during the weekend
- Sunday evening: Sets the tone for the week
Your first dose is 2.5 mg—the initiation dose for GI adaptation, not a therapeutic dose for weight loss or diabetes. Significant effects begin at 5 mg.
The Mounjaro Auto-Injector
Unlike multi-dose GLP-1 pens, Mounjaro uses a single-dose, pre-filled auto-injector. Key advantages:
- No dose dialing required—each pen contains exactly one dose
- Hidden needle—you don’t see the needle before or during injection
- Automatic injection—press the button and the device does the rest
- Audible click confirms injection is complete
The pen is designed for people who have never self-injected. It’s intentionally simple.
Giving the Injection
Injection Sites
- Abdomen: At least 2 inches from belly button (most popular)
- Thigh: Front or outer upper thigh
- Upper arm: Back of arm (may need assistance)
Step-by-Step
- Remove pen from refrigerator (optional: let warm a few minutes)
- Wash hands thoroughly
- Pull off the gray base cap
- Clean injection site with alcohol swab; let dry
- Place the clear base flat against your skin
- Unlock by turning the lock ring
- Press and hold the purple injection button
- Listen for the first click (injection starts)
- Keep holding until you hear the second click (injection complete)
- Slowly count to 5, then remove pen from skin
- Dispose of the entire pen in a sharps container
The two-click system makes Mounjaro's pen among the simplest injectable devices available. Most patients master it on the first attempt.
After Your Injection
- Small blood drop: Normal. Gentle pressure—don’t rub
- Slight stinging: Brief and normal
- No sensation: Also normal—the needle is very fine
- Injection site redness: Common, harmless, resolves in hours

At 2.5 mg, many patients feel very little. Don’t be discouraged—significant effects begin at therapeutic doses (5 mg+).
Common Mistakes to Avoid
- Not holding the button long enough—wait for BOTH clicks
- Injecting through clothing
- Using the same injection site every week
- Skipping meals entirely because appetite is reduced
- Not setting a recurring weekly alarm for injection day
Boxed warning — thyroid C-cell tumors: GLP-1 receptor agonists (semaglutide, tirzepatide, liraglutide) carry an FDA boxed warning for thyroid C-cell tumors observed in rodent studies. They are contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN2). Tell your provider immediately if you notice a lump in your neck, difficulty swallowing, or persistent hoarseness.
